MEDICAL WAIVER & RELEASE

Team Florida Current and any facilities where tryouts, practices, or games will be played will assume no liability for injury or damages arising from the results of the above-named athlete’s participation unless due to the willful misconduct or gross negligence on the part of Team Florida Current, its affiliates, or agents.

Due to the strenuous nature of basketball, the Athlete participating and their parents are urged to consult their physician concerning the Athlete’s fitness to participate. A physical is required to play and should be on file for the athlete at time of request. Basketball presents certain inherent risks and hazards, which the participating Athlete is urged to consider and which the Athlete assumes. To the best of my knowledge, there are no physical or other conditions which will interfere with my child’s participation.

I hereby approve of the participation of my child, the above-named Athlete, in the Team Florida Current athletic program and consent to the emergency medical treatment for my child on my behalf.

TEAM REGULATIONS

I understand that all Athlete’s must try out and play in their birth year or play up in birth year. I understand that all Athlete’s trying out are eligible for the AAU or National level team.

  • I understand players should immediately report any injuries to the coaching staff.
  • I understand players will address the coaches as “Coach” or with his/her last name (example: Coach Mays). I will show the upmost respect in all dealings with coaches, teachers, teammates, fellow students and opponents.
  • I understand players are expected to attend every practice session, meeting, and game.
  • I understand that all missed practices, both excused and unexcused, will hurt the team’s chance of competing in games because they limit time to learn the skills necessary to help the team.
  • I understand playing basketball is a privilege and all players are to always represent your team, coaches and, more importantly, family in an excellent manner. You are expected to behave like a proper young adult and good teammate.

GAME FLOOR REGULATIONS & SPORTSMANSHIP

  • Basketball is an emotional game, and strong emotions are integral to any good basketball team. Losing control of those emotions, however, is not acceptable.
  • Sportsmanship toward opponents, officials and spectators is always expected. In the heat of ‘battle’, people make mistakes. If that happens, we will get you out of that ‘heat’ by removing you from the contest until the coaches deem you have collected yourself.• If losing your composure becomes a pattern, we will develop a plan to improve that situation. That plan may involve a decrease in playing time.
  • Players who receive technical fouls or who don’t show proper respect toward officials and opponents will be assigned to run at practice. Continued problems will lead to missed games and possible dismissal from the team.

PARENT PLEDGE

  • 1Influence my child to abide by the contract they have signed.
  • If my child is having problems, such as poor academics or bad attitude, etc., I will contact the coaches as soon as possible to discuss what is going on so the coaches will be made aware of my child’s well-being and what may be bothering them. I will see if the coaches can assist my child in any way they can.
  • Be an example of good sportsmanship at games by making only positive comments about all players, keeping criticism of the officiating to a minimal volume and treating all fans, and our opponents’ fans with respect.
  • Be patient with coaches who are not giving my child the playing time I feel he deserves. *If you have any questions or concerns, please come to the head coach and discuss how you feel..
